Job Title: Nurse Practitioner – Community Rounding | Wound Certification Sponsored
Company: Twin Cities Physicians
Compensation: $105,000 to $120,000 annually
Benefits: Full Benefits Package (for full-time status)
Location: Greater Twin Cities Metro Area, MN
Schedule: Full-Time or Part-Time

About the Role:
- Deliver mobile, patient-centered care to seniors
- Focus on wound care and complex chronic condition management
- Coordinate with RNs, families, and external specialists
- Participate in interdisciplinary rounds and community huddles
Qualifications:
- Minnesota-certified Nurse Practitioner (AGNP or CNP)
- Wound care training provided if needed
- Skilled at working independently and problem-solving
- Compassion for aging adults
