Position Overview: We are seeking a highly skilled and compassionate Dual Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP) to join our team. This role will involve providing psychiatric consults in a rehab setting and managing chronic care for residents in our senior living community. The ideal candidate will have a strong background in both psychiatric and family health, excellent communication skills, and a passion for improving patient outcomes.
This position is a hybrid role that will work one-two days bi-weekly in person and two-three days via telehealth.
Key Responsibilities: - Conduct comprehensive psychiatric evaluations and provide ongoing psychiatric care and management for patients in a rehab setting.
- Develop and implement individualized treatment plans for patients with mental health and substance use disorders.
- Provide medication management.
- Collaborate with a multidisciplinary team, including physicians, therapists, and social workers, to ensure coordinated and comprehensive care.
- Conduct chronic care management for residents in a senior living community, focusing on the management of chronic diseases such as diabetes, hypertension, and heart disease.
- Perform routine health assessments, diagnose and treat common health conditions, and provide preventive care.
- Educate patients and their families about chronic disease management and healthy lifestyle choices.
- Maintain accurate and up-to-date medical records in compliance with regulatory standards and company policies.
Qualifications: - Dual licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP).
- Master's or Doctoral degree in Nursing from an accredited institution.
- Current and valid Louisiana licensure as a Nurse Practitioner.
- Certification from a recognized credentialing body (e.g., ANCC, AANP).
- Minimum of two years of experience in psychiatric and family practice settings.
- Strong clinical skills in psychiatric assessment, diagnosis, and treatment, as well as chronic care management.
- Excellent interpersonal and communication skills, with the ability to work effectively as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Proficiency in electronic health records (EHR) systems.
