Job Title: Home Health Physical Therapist
Join our team as a Physical Therapist in a home health setting. This full•time, 13•week contract position requires a commitment of 40 hours per week, primarily Monday to Friday, with occasional weekend work.
Responsibilities:
• Deliver physical therapy services to home health patients
• Manage an average of 4 patients daily
• Handle cases including ortho, medication management, wound care, disease education, and elective surgeries
Requirements:
• Valid CA PT License
• BLS certification
• Minimum of 2 years of experience, preferably in Hospital Critical Care or Home Health
• At least 1 year of recent experience within their specialty
• Proactive and eager to learn and contribute
Additional Information:
• No on•call requirement
• All time off must be approved in advance
• Role based in Fresno, CA
• Training on Oasis and Homecare Homebase provided
• Peer support available throughout contract
