Home Health Occupational Therapist (Days) – San Ramon, CA

Contract Length: 13 weeks
Hours: 40 hours per week (flexibility required)
Job Overview:
AccentCare San Ramon is seeking an experienced Occupational Therapist (OT) for a Home Health position. The ideal candidate must have true home health experience with OASIS documentation proficiency.
Key Responsibilities:
- Provide home-based occupational therapy services to patients across Alameda County
- Manage a caseload of 20-25 patients per week, averaging 5-7 productivity points per day
- Complete all documentation same day using Homecare Homebase (HCHB)
- Attend mandatory in-person office meetings once a week (Tues, Wed, or Thurs)
- Maintain a flexible schedule, with potential patient visits before or after standard office hours (8 AM – 5 PM)
Requirements:
- License: Active California OT License
- Certification: BLS (Basic Life Support)
- Experience: Minimum 2 years in home health with OASIS-E documentation
- Technology: Proficiency in Homecare Homebase (HCHB)
